The principle aim of the Diana service is to offer nursing care at home to children aged 0 - 18 years across Leicester, Leicestershire and Rutland through a seven-day service.
Key functions of the service are to:
• Provide nursing care, support and education for children with acute, complex, palliative and long-term conditions
• Terminal care support in the home
• Community-based specialist play, counselling and emotional support to Children and Young People, whilst supporting their primary carer givers and siblings
• Hands on care and an expert resource in children’s/young people cancer care.

Referrals must be received from a professional on a referral form.
All referrals should be made direct to the service using the referral form available
from our website: www.leicspart.nhs.uk (search for Diana referral form)
LPT FYPC staff may send a request for involvement through System One.
• Referral can be made by any professional involved in the child’s care
(e.g. health, social, education)
• Referrals cannot be directly received from a parent/care
