Everything we do is designed to help people regain optimum independent living skills. We are not a service that provides domestic help.
Our staff are trained in helping get back confidence and independence in the home. An individual will be visited (at home or in hospital) and together goals are agreed and support is identified as to how those goals are reached. Goals can be around; meal preparation, managing medication, washing and dressing, bathing or showering and mobility or accessing the community.
These goals will be recorded in an individual’s Reablement Plan, and the goals are reviewed on a regular basis and adapted to fit in with progress made.
At all times the persons views are listened to and respected, and any changes will be discussed.
If we feel a piece of equipment is needed to support with safety and/or independence we can arrange for this to be to be delivered on loan or discuss options for purchasing privately or through a Direct Payment.
Support workers will visit during the hours of 7am and 10pm at call times which have been agreed, to help the individual get back to their base level. This could be up to 4 calls a day dependent on what is needed .We cannot give an exact time for each visit but will work around an approximate time.
There are Occupational Therapists, Physio Therapists and Community Nurses who are part of the team, to support with equipment, exercises and other medical issues that don’t need a GP or to support a GP.
We can support by signposting to the relevant services if required.
