Transition - Preparing for Adulthood
SEN & Disability Local Offer
This section of the Local Offer is all about health services available for children and young people with special educational needs and their families.
You can find out about services available locally as well as national information, advice and support.
You can use the subject buttons below to find information, or search the service directory.
Transition happens between the ages of 14-25.
This is often called preparing for adulthood.
To make sure the transition is smooth, early planning is essential.
Some young people will be eligible for adult care and support services.
If you don't qualify for support from the Council, you can still get information and advice about other support services you could use.
The Local Offer is a free service provided by Rutland County Council.
Being listed in the Local Offer does not mean that we recommend or endorse of any of the services.

August 2016 - based on some amazing feedback from the Rutland Disabled Youth Forum we have added in more categories for information, to make it easier to find what you are looking for.
We also received feedback from parents to add logos for services in, to make the site more visually appealing. We have done this where possible.
